Cost and Insurance
For clients using insurance, out-of-pocket counseling costs will vary according to your insurance plan benefits. Contact your insurance company or consult your plan documents to obtain details about the benefits of outpatient counseling in an office setting. For clients who wish to self-pay (not use insurance) at the time of the visit, the cost is $75 per visit.
Methods of Payment
Payment may be made by cash, check, or credit card. Payment is expected at the time of your session. For more information about how to make a payment, view the make a payment page.
Insurance and EAP
At this time, Radzom Counseling is a participating provider for the following EAP and health plans (please note that not all providers within the practice accept the same insurance plans):
- Beacon / Value Options (insurance plans; not generally for EAP)
- Blue Cross/Blue Shield
- E4 Health EAP
- Morneau Shepell / Chestnut Global Partners
- MHNet (behavioral health provider for Coventry)
- PAS (Personal Assistance Services)
- TriCare (most plans)
- United Behavioral Health (United Health Care / OptumHealth)
For clients with other insurance plans, the practice is frequently able to provide the documentation that your insurance company requires for reimbursement of out-of-network services rendered.
Documentation and Reports
Generally speaking, our counselors do not create or provide documentation beyond ordinary medial records. The creation of reports, assessments, treatment summaries, letters, or other materials that extend beyond the request of ordinary medical records are billed at a rate of $90 per hour that is spent creating, writing, processing, and delivering the documentation. Fees for documentation may be waived at the discretion of the counselor and the practice. Insurance plans do not provide coverage for these fees. Some EAPs prohibit counselors from providing reports to clients. We are not able to honor every request for documentation or reports. If you want or need documentation, we encourage you to discuss this prior to scheduling so that we can verify that we are able to meet your needs.
It is understandable that appointments need to be canceled and rescheduled on occasion. It is required that you give at least 24 hours advance notice when canceling an appointment. This is because the practice needs time to be able to make the appointment time available to other individuals. The practice reserves the right to charge $45.00 for sessions that are canceled without 24 hours of notice.
